Abstract

To the Editor: The report by Gold and colleagues (May 22 issue)¹ indicated that the plasma ACTH response to ovine corticotropin-releasing hormone stimulation can be helpful in the differential diagnosis of two conditions of hypercortisolism — the depressive phase of primary affective disorder and Cushing's disease. This conclusion was based on the finding that the response to ovine corticotropin-releasing hormone appeared attenuated in patients with depression and exaggerated in patients with Cushing's disease. In particular, the “peak ACTH response,” defined as the highest level of plasma ACTH occurring within one hour after a bolus injection of 1 μg of ovine….
